The Reachout Project is a Non Profit organization that depends on grants and donations to enhance the prevention of Chronic Illness in our communities so that we may detect it early enough to slow progression through advanced prevention care or prevent it from ever happening. Chronic Illness has plagued many communities throughout the United States. Unfortunately due to the lack of basic healthcare education, improper diet and lack of treatment options this preventable disease impacts minority & low income communities disproportionately compared to others. Initially a research program targeted towards the local communities, Colleges and Universities in the Florida area, to a nationwide effort. Health Care, Education, and Economic growth are all directly correlated and we will provide the platforms, programs and initiatives to accelerate that growth.

By providing medical, pharmaceutical services & Preventing Chronic illness or detecting it early to slow progression it significantly decreases illness and mortality in our communities.
This is done through advanced prevention care, academic excellence, research superiority, and community service. Our “it takes a village & prevention is the secret to wellness” approach to reach out will also prepare local community students for leading roles in the practice of research and healthcare.
Ensuring the overall health and healthcare outcomes of families, with a special emphasis on the chronically ill, as well as health equity for the vulnerable and underserved populations.
Healthy Kidney Hubs will be established in Local Community Churches, Health Clinics, Assisted Living Facilities, Retirement Homes and Active Adult Retirement Communities throughout the US in partnership with current ongoing initiatives.
The Healthy Kidney Hubs will facilitate education seminars, exercise classes, meal plans, urine tests and blood test.
The Healthy Kidney Hubs would also monitor program adherence, and the progress of the patients Kidney health.
These Healthy Kidney Hubs will include periods staffing of healthcare professions such as pharmacy students, medical students, nursing students, dietary students and personal trainers who would all be supervised by licensed medical professional staff.
Studies have show that the simultaneous presence of two or more diseases or medical conditions in a patient, such as diabetes and high blood pressure, play a role in the development of chronic disease and impacts minority communities at high rates.
Crafting healthy lifestyle guides which include diet, weight management, exercise, monitoring alcohol consumption, smoking and blood tests all play a critical role in prevention and early intervention of Chronic diseases.
